    Heres the situation.
Marine Corps recruitment is happening for me at the end of this year(December 2011) when I turn 17, and I am literally in the worst shape of my life. My goal is to reach at least Outstanding(Low) - see chart below - by that time so I will be ready for basic training.

At this current moment here are my statistics:
    • Run Time(1.5 Miles) - 12 Minutes
    • Curl-Ups - 50
    • Push-Ups -40
    • Pull-Ups - 16 (Not required by Navy, but is within my JROTC unit)
    • Swim -N/A

Taken from the NROTC Physical Fitness Requirements.
    * Amounts without time limits are assumed to be completed in 2 minutes.

Does anyone have any tips about creating a physical fitness plan to help me reach this goal?

Everything you do gets easier over time. In a week you'll be able to do the push-ups part, as long as you exceed what your last mark was. Keep it in your greatest priorities to do this, because when you get to the Marine Corps I can only assume how much harder it will be especially that you're only in the satisfactory level. I run every second day (5miles) and I can honestly say that at first it was extremely hard, now I can easily do it and want to run more than 5 miles or even more often and set new goals. Also, another tip: Keep a routine going, if you stop exercising after you have a system going it's hard to get back in, but it's also hard to get the system going .

Sadly for me, in Canada there's no marine corps, although there's some sort of training for that. I'm 16 but plan on joining the army when I graduate high school(only for ~3 years, to get $ before I go to college and to experience it.), just don't know what yet. Good luck with it, sounds exciting.
